2. Report to ESI Authorities / Inspector
3. Submission of accident report form to ESI Local Office

Report is to be sumit to the following:
i. Inspector of factories;
ii. ESI Branch Office within 24hours from such incident;
iii. Incharge Medical Officer, ESI Dispensary connected to such employee.....

Further Last date for monthly contributions is 21st of the following month;
There will be two half Yearly returns, one is for the period ended by 30th September(april-september) & another is for the period ended by 31st march (october-march). The due date for them are 11th November & 12th May respectively.
One Annual return in the Form-01(A) to be submitted every year before 31st January.
As of now, ESI is to be paid on all components i.e. BASIC, DA, ALL ALLOWANCES(except Washing Allowance).

can any one please explain the practical case if an accident takes place in any company? waht is to b done apart from above mentioned intimations and filling the form 16, and remit the same before nearer and applied dispensariy and esic office (regional). in a returns of that we may get certificate of fitness given to ip by doctor, and the same is to sent before regional office of esic.
how long will it take to make the payment to the person who has claimed for that benefits?
please give me an idea of the same along with the percentage or any amount or how much reward would ip get at the end of this procedure?

Dear Seniors,
One of our old employee got road accident on the way of her hometown (Kanpur) and she need to be hospitalized in esic hospital of Kanpur, but there ESIC People are not ready to admit her due to requirement of some paper. So, can any respected senior help me out on this matter urgently, so that I could help her from our office? Because this the first case of this type and even I don’t know what formality I have to do? Thanks for your kind help..

Utmost at the Dispensary they may ask ESIC-37, ie.. Certificate of Continuous Employement; in case need of Emergency Certificate they may ask the Eligibility Certicate from the concerned Branch manager....now a days due to computerisation status of the both the things can be viewed by using ip portal which can be available in the esic portal under caption "IP Portal" at the bottom left corner of website homepage of "www.esic.in", But this can work only on Mozilla Firefox only.
In ur case i strongly feel that they may required ESIC-37 and his Permanent Card, if not available Temporary Card.
Wishing U all the best..................One more small suggestion...Every HR should have a list of ESI Dispensaries; Hospitals and Private tie up Super Speciality Hospitals within their state....
