The format is very good.
The only suggestion is - if we are going to give one page dedicated for each employee, the name of the employee can be mentioned at the top of the page insted of having it in the 2nd column.

My only 6 months experience as HR Assistant, I have responsibilities for Expatriates, National Employees' Attendence and collecting monthly timesheet, Organization Chart Updated, key in to Employees Information if anything change, Leave Management for Expatriates and National Employees, Salary calculation ( OT, Meal Allowance, Transportation Allowance) for National Employees, Salary Calculation for Contracted Employees, Assist for Monthly Man Power list to Head Office, Performance Evaluation for National Employees, Annual Bonus Calculation for National Employees base on their PE, now preparing for Annual salary increasing proposal etc... So I feel like HR Assistant means lots of Calculation and Excel.
I have other responsibilites from my job description such as need to assit for recruiting National Employees, Orientation for New employees, ensure local laws and legislation are followed and many many more...
And if my HR Manager is out of office, I have more and more responsibilities...

hi,
most of the information in company is confidential... but OT rate is simple... we have OT1 and OT2. OT1 means weekday overtime, OT2 means weekend and holiday overtime.
R= (Monthly Basic Salary x 12)/365
Regular work hours/day (8)
OT1= time worked in excess of 8 hrs on regular work day ( depenes on Company Policy% x R)
OT2= time worked on weekend, holidays ( depends on Company Policy% x R )
Trip Allowance is simple too... daily trip allowance is just followed company policy ( how much per/day) except accomodation + traveling charges+ entertainment charges + no bill expenses etc)
Meal Allowance also very simple .. but need to follow company's procedure. Here even different calculation for Expat and Nationals.
Performance Evaluation is based on department Manager and HR judgement which can increase their salary and also Training requirement.
I am sorry that I cannot share our forms even we have so many forms here. Trip Form, Leave Form, Timesheet forms, Performance Evaluation Form etc.. However, you can develop these forms from your idea.
