I guess, you do not intend to copy any profile doc, but need a format to refer the content and its sequence.
First thing search Google. First thing search Google. Ook for models and formats. Do not copy the content, it shall not help your purpose.
Second, just go through this suggestion.
The Cover page punch line should be a phrase or a sentence which reflects your activity and its distinction from the crowd which could give the reader reason to read the rest.
Then comes your vision/mission/philosophy followed by the people at the helm and the team, resources(office, location, equipments etc. as applicable)
Continue with details of what you are doing and what 'qualifies' (read Strength) you and your claim of distinction.
Follow with list of clients, success stories, case studies, testimonials etc. to reinforce your claim. (Here you need to lead the reader to perceive the promised benefit in terms of value he/she is likely to expect/demand.
Conclude with "demanding action" i.e. request to contact you, call you, etc.
That's it.
Looks simple, no? But it's not. It's not so difficult to write about others, but when it comes to write about oneself the game gets difficult.
Always prepare a draft, keep it aside. Review later wearing the shoe of the prospect. YOu would find refining opportunities.
Get a close confident or associate to read and comment.
