Hi
I have been asked by my manager to prepare a 360 appraisal questionnaire, I am not able to get the right format for the same, thus would like to know the basic format for such a questionnaire and the process of rating and evaluation.
Would appreciate your help on this!!!

The 360 instrument we supply to our 50,000+ clients that want a 360 has 70 questions. The 360 developers started with more than 1,000 questions and worked for years to reduce the questions to a manageable number and to fully understand what each answer means.
Employees' rating forms should not be seen by other employees including HR, the boss, the CEO, etc.
Are the employees prepared and willing to offer honest feedback?
Are managers ready to accept anonymous feedback?
Is the employer prepared to train and coach managers to change their behaviors?
Doing a 360 is the easy part, getting managers to change their behaviors is the hard part.

360° Feed Back is common method of Performance Appraisal, in which you usually ask the Peers, subordinates, managers of that employees about the conduct of the specific employee.
It contains number of questions, e.g. The strengths of Employee, his/her behavior, communication and interpersonal skills, weaknesses, targets, how well he define his targets and go for them.
Then other questions may include to rate some skills of that employee by his peers, subordinates, boss etc. like Communication skills, Interpersonal skills, organization skills, time management skills, and other…
Then the employee is rated on the defined scale….
