i want to write a letter to our college&School to inform them that my certificates missing.Please help me how to start a letter

First thing first you have to give one advertisement in the Newspaper to this effect or in the alternative be required to obtain a duly attested affidavit by First Class Magistrate in this regard.
You may start the letter as under :
BY REGISTERED POST/COURIER/BY HAND

Subject : Intimation about loss of certificate and request of getting issued duplicate Certificates.
Sir/Madam,
I was a student in your esteemed institution in the class _____________ w.e.f _________ to _________________ having roll number/enrolment number __________ . On account of circumstances beyond my control I have lost/misplaced my qualification certificates (mention numbers and dates if any) hence this application.
Kindly accord your approval to the concerned person in this regard so as to enable the undersigned to get the duplicates on ASAP basis.
