Dear Anju,
This is really nice and will help quite many HR professionals emphasizing on PMS. However the only problem I see in the form is the inclusion of too many competencies/behavioural traits.. In any robust PM system is is advisable that u dont have more than 6-8 KRAs and not more than 8 competencies. because it then becomes difficult for any individual to display those many positive behaviours..

Very nice and very useful but it is too elabrative can you make it small so that management should have patience to go through it and in filling it. Otherwise it is simply good.
