Dear Friends, Required is a format of annual returns under maternity benefit act i.e. i guess we need to submit on or before 31st jan every year. Can anyone share me with the form. Jagdish V:icon1:

The annual return in forms L, M and N to be furnished by the employer under Maternity Benefit Act are to be filed on or before 31st January every year.
The forms are attached for reference.

Dear Mr. Madhu.T.K,
You are right my subject does not match with the content. It was a typograpical error. I am sorry for that.
Anyways thx a ton for the forms (annual returns under MB Act). Plesae clarrify me with the following:
1. The female employees under MB act does it cover contractual female employees (Housekeeping, gardening)
2. The total strengh of female employees directly employed on the payroll is 14 No's., which i guess is much bellow the limit i.e 30 no's. Please correct me if i am wrong. So here in this case do we need to file the retuns even if the total female employee strengh is bellow the limit.
3. Are all these 3 forms to be filled seperately or they are together.
Please respond to it. I am sure u will guide me on this

If the house keeping/ gardeners are employees whose names appear in your muster roll and EPF/ ESI are deducted and paid by you, they are entitled for maternity benefit.
Maternity Benefit is payable even if the company employs only one female employee.
All the forms are to be filed together.

is it mandatory to file annual return (form L, M ,N, O) under maternity benefit act when esi is applicable. there u few females who are out of coverage of ESIC but if anyone has not taken maternity benefit do we still need to file "NIL" return under maternity benefit act.

Dear all,
Please note that ESI Annual Returns has to be filed before 31st January 2013. Annual Returns is the information you have to provide with regards to the Employer only and employee details is not required.

Dear Gangadhar ji,
You are very correct. According to regulation 10C every employer in respect of a factory or establishment to which this Act applies and to whom a code number has already been allotted, shall furnish to the appropriate Regional Office or Sub-Regional Office or Divisional Office, by 31st of January every year, a return in Form 01(A). The employer shall be responsible for correctness of all particulars and information furnished in Form 01(A).
Please find attached a From 01(A) for ready reference of all the members.
