Hello Dhruvin,
I think the content for this letter, are his/her duties and resposibilities, expectation of the company towards the given opportunity, it may add incentives depends on the company...
This just alittle info, hope it helps. Thank you

Please note that in Offer letter / appointment Letter you need not to give all the details about the company policy. But you must give the details about the Compensation Package, Benefit Package, Working Hours and days, Discipline matters and separation procedures. For other policy matters after joining the organsation, the HR Head can give him a copy of Hand book of Office Policy and obtain a signature for "read and understood the HR Policies/Office Policies and will follow the instructions given in the above" will serve your purpose.

Dear Dhurvin,
Offer letter should have the offer for the particular job position for which the person has got selected and the annual salary that is being offered or you can also mention the date of joining on the offer letter.
Whereas appointment letter is given once a person accepts your offer letter and joins the organization. The appointment letter should contain the complete information regarding the employment of the person with the organization.
E.g.; you can start from probation period, reporting, job location, termination clause, notice period, complete salary break up etc.... and all the terms & conditions of employment that you want an employee to understand.
I have attached samples of offer letter and appointment letter for your reference.
