Hi,
I need a letter format for Temporary Break-up in employment due to illness ( Medical Treatment) for short term. The details are as follows,
1) Employee is on confirmed from 2 months.
2) As per medical Report he has been advice for medical treatment for 6 months for infectious disease as it will be harmful if his present in the premise.
3) he can join our firm after treatment completion as per the medical officer's certificate.
Plz help me.

The aforesaid termination shall come into effect from _______ 2009.
You will be allowed to join your work after the six months i.e. _________ (subject to the recommendation of medical practitioner).
You are requested to carry your medical certificates at the time of resuming your work after the noted period. All other terms and conditions of your employment remain unchanged.
