Under which of the Act & Rules you are requiring this form V. In general Principal Employer under The Contract Labour (R&A) Act & Rules issues form V to the contractor & for this also first see which of the State or Central Rules are applicable in your case & then go through those applicable Rules to have the format of this form.

Form V attached by one of our member relates to units where The Govt. of Karanataka is the appropriate Govt. while in your case you should utilize the form V provided by the Gujarat Contract Labour ( R&A) Rules, 1972. According to the Labour Department site of Gujarat form V may be read as under:

Form of Certificate by Principal Employer
Certificate that I have engaged the applicant (name of the contractor) as a contractor in my establishment, I undertake to be bound by all the provisions of the contract Labour (Regulation and Abolition) Act, 1970, and the central Labour (regulation and Abolition) central Rules, 1971, in so far as the provisions are applicable to me in respect of the employment of contract labour by the applicant in any establishment.

After going through the format of this Form V it is on the same line as provided by the Central Rules of 1971. In my view in place of central rules it should be as follows:
The Gujarat Contract Labour (R&A) Rules, 1972.
