Hi Rahul,
We thank Syed for providing a template for self appraisal..it's good one..
Let me share the pros & cons of the same..
Pros:
a) Employees feel they have a voice in the process.
b) Provides ability to report progress toward goals, including challenges and roadblocks.
Cons:
a) Individuals tend to rate themselves harder.
b) Individuals may feel that managers do not know what they are doing.
It would be better if you provide the column for supervisor to moderate the same.

Hello,

Following are a few parameters you could look at in a Self Review. I have also mentioned what you can look for when considering these parameters. This is what I have designed for my company. It might change from company to company. So you can take whatever is applicable to you.

PARAMETERS

Inter-Personal Skills

The degree to which the employee takes time to develop and mentor others

Ways to promote your interpersonal skills:

• Share your knowledge and information with others

• Promote honest, confidential sharing

• Give feedback to others

• Counsel, coach, and encourage others

• Meet with your team members regularly

• Assist your subordinates in developing their talents

• Maintain objectivity and balance in your attitude

• Allow your team members to grow and become more independent

• Foster a sense of risk-taking and independence

Planning and Organizing

The degree to which a member plans and organizes goals for the team and company

A member who can be rated high in planning and organizing is one who:

• Prioritize duties in a manner consistent with organizational objectives

• Effectively manage tasks or program assignments including follow-through and delegation

• Able to identify the needs of the team before a major change

• Able to look ahead (beyond the present) when addressing the work/needs of the team.

• Anticipate resources needed to successfully implement a plan or project

• Work toward identifying potential problems and solutions for addressing them

Decision Making

The degree to which the employee makes decisions and acts on those decisions in a timely and efficient manner.

A member with a high level of zeal for action is one who:

• Is not afraid to take corrective action when necessary

• Make effective decisions, even when under pressure

• Effectively make decisions

• Display high energy and enthusiasm on consistent basis

• Work quickly when faced with difficult problems

Commitment

The degree to which a member is committed to working towards company goals and values

Methods for being committed include:

• Set clear goals along with the members that require efforts to achieve but are reachable

• work towards accomplishing organizational goals

• involve yourself in decision-making processes that affect the work as well as the quality of work life

• using inputs from your team members and team leads to develop your performance plans

• create a work climate for yourself where team members feel valued

Team Work

Works effectively on teams by actively contributing to the accomplishment of goals

Why work as teams?

Good teamwork achieves synergy and enhances performance. Encourage others and value each member's contributions.

Innovation

The degree to which the member creates solutions to problems and finds new ways of working

Ways to be innovative

• Bring up good ideas at meetings

• Create new and imaginative approaches to work-related issues

• Find creative ways to get things done with limited resources

• Show a willingness to question traditional assumptions

• Improve on ideas of others

• Use "unconventional" approaches to solve customer and business problems

Self

The manner in which an employee conducts his or herself in the working environment

A member with a high level of values and ethics is one who:

• Display professionalism

• Demonstrate effective time management skills

• Show positive attitude

• Meet the productivity standards

Performance

The degree to which the employee exhibits high performance and success in the work place

Methods of achieving high levels of Performance

• Align team agenda with strategic corporate goals

• Optimize processes and lower costs

• Monitor results in real time

• Increase customer retention

• Set ambitious goals and means to achieve them

Technical

The degree to which the member possesses the technical knowledge and skills necessary to succeed in the workplace

Methods of Improving your Technical Skills

• Increase your knowledge on programming skills

• Choose appropriate conceptual and qualitative models

• Identify ambiguous or missing information

• Research information from a variety of sources

• Maintain skills through professional development and training
