So let us not call / name / categorise it as a "mistake" rather term it as a "gap" or "deviation".
If a gap / deviation is identified, then the root cause behind the same has to be analysed through a brainstorming with all the concerned persons. You have to arrive at appropriate corrective action along with the target date for implementation of corrcetive action taken. And the same has to be monitored on a regular / periodic basis depending on the type of gap / problem to avoid its recurrence.
