I am in urgent need of a FILLED Form-L for the applicant employee, nominee, or legal heir.

Form L means? Under which Act you mean? If you mean Form L of Payment of Gratuity Act, the same is a return to be sent to the Appropriate Authority under the payment of Gratuity Act and it is not a form to be submitted by the employee.
