Hi, Our organization gives the employees option whether they would like to go for PF contribution or not.
In case the employee is not willing to opt for PF we need to take a declaration. Can anyone guide me is there any PF form for declaring the above thing.
Need assistance.

Dear Raja,
Please note, If PF once deducted for the employee it has to be deducted even-though salary limit is over and above the prescribed limit. The above given option is only available for the employees who were never been a member for PF and there Basic is over and above 15000/-

Manoj Kamble is right, you cannot opt out from PF just because your salary has exceeded Rs 15000.

If Rajeesh is not an existing member of EPF and along with that salary at the time of joining the company is more than Rs 15000, then only you can become an excluded member of PF. For that you have to give a declaration in form 11. Form 11 is a declaration of exiting PF. If you declare that you do not have PF account, then you may be excluded from PF by the employer. Normally this form will be given to you by the HR person carrying out the onboarding processes.
