Unfortunately I lost my EPS pension Scheme Certificate received earlier from EPFO. Upon enquiring I understood that an affidavit cum indemnity bond has to be submitted to EPFO for re-issue of Scheme Certificate in case it is lost or mis-placed. I request you to kindly share the format that is to be used for the affidavit cum indemnity bond for this purpose. Kindly also let me know what is the value of Stamp Paper (Stamp Duty) to be used to print the same. Thank you for your help.
