Iam working as an engineer in an electronics manufacturing company.An appraisal form is given to me which includes some questions for me and to my immediate superviser.i dont know how to fill it well so that i get a fgood appraisal.
Questions includes
1)Summary Of achievements(Briefly explain your scope of work during the review period with specific reference to targets and achievements)
2)Explain any specific reason which had inhibited your performance?
3)Whether any change in your present assignment will improve your performance?
4)Whether you are satisfied with your present level of knowledge of work or any further training is needed?

First things first
On the first look to your query you seem new to the industry.
One cannot fill good appraisal. You need to be really really honest irrespective of what your boss feels. If you have worked hard you will for sure reap the benefits in the long run.
1. Make a matrix for yourself where you tabulate the goals given (in writing) vis a vis your achievement. To avoid the stress of remembering what you worked for the entire year. It is always better to maintain an excel sheet weekly to update the work accomplished. This will do wonders. Start from now on. This will help you in future.
2. Its not necessary that you have not accomplished. Simply eliminate the column if it is genuine by writing NA (not applicable).
3. You think for yourself whether you need a change and then answer.
4. The best answer to give is according to me is "there is always a scope for improvement and learning never ends". Specify if you need to undergo any specific training and how the organisation can benefit from it.
Remember whatever your answers are always be honest to yourself. This will speak volumes. Let your work speak for itself and not the answers in the appraisal form.
Don't be afraid to mention your company name...
