You have to file Form No.4 along with Form No.5 which will be issued by the principal employer to the Labour department seeking licence. K.S.Srinivas

You have approach Inspector of factories applicable to your area along with the above said fomrs. You have to deposit some amount as per slab rate and he will give you contract labour license.

In which type of industry you belong to. Please give some more details about your requirement.
Normally the contract labour licence is issued only to the contractors who are those engaged by the Principal Employer who got Certificate for Registration of Establishment under Contract Labour Act.
Please fill Form IV (Application for LabourLicence). Obtain Form V (Certificate from Principal Employer) from your Principal Employer. Attach copy of Work Order issued by your PE. Pay the Labour Licence Fee and Pay the Labour Security Deposit and attach both the Original Challan alongwith the Application. Then submit the same for obtaining the licence. (Please keep the copy of all the documents with you as this challans are required by you while surrendering the licence and get the Security Deposit back).
