well there can be more than five parameters of PA as it depends upon the process.
Broadly you can distinguish ithem into tow One is Skill based which is directly related to the job/Process.
Secondly is the behavioural aspects which helps an employee to perform that job or succeed futher in his career.
You can break down the Skill part into like:-
knowledge , quality, feedback etc...
Behaviour will inculde..like Team leader qualities, motivation level, discipline, planning & management etc......
