After a long time, I need a favor from all of you, especially those who have done some research on Employee Motivation Surveys. I need to design an Employee Motivation survey very urgently for my organization. I want to know what things we need to take care of while designing the questionnaire, the evaluation procedure, and the criteria on which these levels could be measured. Also, what measures should be taken care of if the motivation of the employees is found to be below the satisfaction level.

After that, the questions should mostly be closed-ended to ensure easy and accurate analysis.
For better results, consider analyzing the data using SPSS (Statistical Program for Social Sciences). Keep the questionnaire concise, preferably not exceeding 20 to 25 questions for better analysis purposes. Responses should be measured on a scale, such as a 2-point scale (yes or no) or a 5-point scale (Excellent, Better, Average, Poor, Very Poor).
