All companies register their employees on the ESIC portal online with all their details and issue an e-pehchan card to them. Is it mandatory to fill out the manual ESIC Form 1 declaration form for employees and keep that form in the personal file? Is there any notification regarding keeping the manual ESIC declaration Form 1 or not? Please suggest.

Not necessary. It is usually filled to help in online registration. Once the e-Pehchan card is generated, the same has no relevance. It is always better if the person in the Personnel department does it in front of the employee at the time of his joining itself.
