Form 2 needs to be continued as regular, This is the additional form required to be collected from the new joinee with the UAN details if any,

See Form 11 and this declaration form are the same in paragraphs 24 and 34, with the addition of 57. So, I think there is no need to collect both declarations; one is enough.
