Form c11 will be issued at the time of your co ESI registration and while applying for subcode they will ask the copy of C11, pls have an enquiry at ESI head office you will get a better picture on what forms to to be submitted for getting a subcode.:)

However I had one more doubt; Can the contributions already paid at the Head office for all employees located in various branches be transfered to respective branches ESIC after getting the sub code or do I have to pay them once again.
