Am looking for Sr. BDM/Sales manager appointment letter template with terms and conditions regarding the targets to be achieved.
I want to mention in the appointment letter stating "the fixed salary will depend on the target achieved on monthly basis", or suggest me the best way to mention in the appointment letter.
Any template will really helps.

You are looking for the template for the appointment letter for the post of BDM. It may be noted that the appointment letters do not differ from one position to another. The format is always standardised. Therefore, maintain the same format across all the positions/designations.
Coming to the insertion of the clause of the salary. In the appointment letter, you wish to include the clause "the fixed salary will depend on the target achieved every month". If you issue this clause in the appointment letter then it implies you wish to convert even "fixed salary" into "variable salary". But then what left to be called as the "fixed" component in the salary?
Please note that you must pay the minimum wage irrespective of the position. The variable component can be on and above the minimum wage. You cannot link the entire salary to the target achievement. If the employee fails to achieve any target even then also he/she remains eligible to get the minimum wage.
By the way, why do you ("you" means "your company"), wish to include a clause wherein even a fixed salary also is required to be linked to the target achievement? Does this mean that you are unable to get quality candidates and earlier your company had to terminate employees on account of non-achievement of the target and your company has the frustration of losing revenue because the recruited employee could not bring business worth of his/her fixed component of the salary?
In that case, it could be a recruitment problem. Going further it could be a problem caused due to your company's inability to attract the right candidate(s). If the malaise lies in either of the two reasons or both then it cannot be cured by insertion of the clause on the non-payment of the fixed salary. The problem could persist even after the insertion of this clause. In that case, you could be back to square one.
