According to my knowledge, there is no specific form or format for the intimation of a change of occupier similar to a change of manager under the FA. You need to write to the authority about the change of occupier on your letterhead with justification for the change/appointment of a new occupier.

You are advised to approach the authority. The authority will guide you according to its rules and procedures. Rules and procedures change from place to place, office to office. Sometimes, you may find a change from one officer to another within the same office.

If the Occupier is also one of the Directors, please attach the list of directors.
