Please help me to ask my boss for a salary increment. Actually, in July 2017, I met with an accident due to that, I had resigned from my job. Again, in July 2018, I rejoined and it has nearly been 1 year now. Before I resigned, my salary was 15,000/m, and they are still giving the same. I want to ask my boss personally.

With all due respect, I humbly would like to request a salary increment. As you know, my salary has remained the same since I rejoined. I kindly request you to consider giving me an increment in my salary. Thank you for your time. I hope to hear positively from you regarding my salary increment request.

Instead of writing an email, meet your boss personally. Before you go, do a bit of homework, like highlighting your achievements (providing data to support why you should be given a raise). Most importantly, meet with the person in charge when they are in a good mood.

As Amby mentioned in the post, draft your achievements in the appraisal period. If they are really above expectations, go ahead and talk to him over the phone. If he responds positively, act accordingly and wait for it.
