Not from 4 (As told by Mr. V. K. Sharma), but form 2, three copies (if not changed) with the original payment receipt of the bank and the original license of last year (if the establishment/factory is already registered), should be sent to the Deputy Director of Factories in your area before December 1 each year.

I am working in MP, and as per our state rules, it is Form 4. As per your state rules, it can be Form 2 and it can be different in that state where Mr. Pandit is working. It would be better if we may know the correct name of his state.

The contents of the renewal form are almost the same, but its numbering is different, which may be form 2, 3, or 4 depending on the State Factories Rules. Moreover, now a few states are handling the renewal process online.
