A few days ago, I lost my original ESI Challans. Now, I need to file the half-yearly report at the ESI Office. Can anyone suggest what I should do and whom I need to meet?

please you go to the bank and tell him to this metter bank show their record and make neceray note against your chhallan paid in our bank

Dear Vinay,

Please prepare a letter on the company letterhead addressed to the bank (in favor of the bank where you have deposited the challan). Include all the necessary details such as the cheque number, amount, date, etc. Upon submission, the bank will provide you with a certificate of ESIC contribution, which can be used as a challan anywhere needed. Please note that the bank will charge a nominal amount for this service.

With addition to the above-mentioned suggestions, also make a habit of scanning all the original documents from time to time (as soon as you receive them) and keeping them in separate folders. Additionally, ensure you have a good quantity of photocopies of the documents as it will help you in times of urgency.

Hi Vinay,

Please go and approach the bankers where you have deposited the ESI challan along with your cheque particulars like cheque amount, cheque number, cheque date, drawn on which bank with your covering letter, and they will do the needful.

Dear Vinay,

Don't worry about it; just approach the concerned bank where you have deposited the amount (cheque). They will provide a certificate stating that you have paid an amount of Rs......... into ESI A/c No. I on your ESI Code No............ towards the contribution for the month of............

A copy of the same may please be attached in lieu of the lost challan. Furthermore, two things you have to do:

i. Write a letter to the Regional Office concerned along with the details of the lost challans and enclose the Bank Certificate.

ii. You can keep your Bank Statement, where the amount is debited towards ESI Contribution (this will be one kind of evidence from your records).

You HAVE TO GO TO THE BANK WHERE YOU SUBMITTED THE ESI CHALLAN, ALONG WITH A COPY OF THE BANK STATEMENT, A COVER LETTER FOR THE REISSUE OF THE ESIC CERTIFICATE, AND AN AUTHORITY LETTER. THEY WILL CHARGE APPROXIMATELY RS 150 FOR EACH NEW ESIC STATEMENT. YOU NEED TO CONTACT THEIR CLERK WHO HANDLES THE CHEQUE CLEARING.
