This is Abhilasha, I am working with one small IT company with employee count of 40 and I am only person responsible for all HR activity in my company.
I need HR people help on 1 new arise issue. we are planning to hire one of our ex-employee, who is joining us after 2 years break. We have already given her experience and reliving letter at time her leaving with company. My question is, again we need to issue her new Appointment letter with all company define terms and condition? and can we give her earlier employee code or need to give her new employee code? If someone is having any format for re-hiring Appointment letter, Please share.

I understand that you are a learner and need to understand that the employee cord allotted to one cant be issue again to anyone no matter is new or old/ex employee.
Though, she is an ex employee but she is now with advance with her skills, knowledge and experience and also it is not necessary that the conditions of employment will remain the same for years.
You must treat her as your new employee and need to issue a new appointment letter, employee code and need to create a new file with her updated documents such as her present experience cum relieving letter (For the period she has spent with other employer then you till date), copy of updated CV, Contact and present communication and other statutory required docs.
And about appointment letter for this employee, as I told you above that she now must be treated like a new employee, you can use your present appointment letter format. An old/ex employee can get many internet/external benefits but there should be no lacks in following the procedure of hiring an employee.
About issuing her a new EPF/ESIC code/no, hope you are aware of the updates from the department. You can use her present EPF number with the employer as the transfer of it is also available. for more need to visit "Official EPF Portal"
in short : treat her as a new employee and complete the task.

You are hiring an ex-employee for different span of employment. Every span of employment is different and different spans have different contracts of service.The service conditions might have also changed as the learned member Mr.Anil has opined Therefore you need to treat her as a new hire and issue her afresh appointment letter.
