Dear Siva Prasad,
Every employer of an establishment/shop shall, within thirty days from the date on which the establishment commences its work, send to the concerned Inspector a statement in Form-I containing the necessary particulars, along with a challan to support the payment of fees as prescribed:
1. Shops/Establishments employing no persons - Rs.30/-
2. Shops/Establishments employing up to 5 persons - Rs.100/-
3. Shops/Establishments employing 6 and above, up to 10 persons - Rs.200/-
4. Shops/Establishments employing more than 11, up to 20 persons - Rs.350/-
5. Shops/Establishments employing 21 and above, up to 50 persons - Rs.1000/-
6. Shops/Establishments employing 51 persons and above, up to 100 persons - Rs.2000/-
7. Shops/Establishments employing 101 and above - Rs.2500/-
