For someone pursuing a PhD degree, I suggest searching the web, and you will find some posts on both CiteHR and others. It did not take me 5 minutes to find the HR Audit Questionnaire at HR Audit Questionnaire and http://www.opm.gov/studies/title5.pdf by Googling on Google.
