ESIC Form 16 is the accident report that must be filled within 24 hours of the accident. It is only required for employees who were injured during work hours and not otherwise.
Form 16 includes all information related to the accident occurrence and its causes, as well as details of any witnesses to the accident.
Form 12 is an application for sick leave remuneration.

No i think it is wrong. now the form 16 is known as form 12 as i have seen that both have almost same format and regulation 68 is mentioned in both. Please confirm me if I am correct. Thank you

You are right, it was before that Accident Report was Form 16. But still in the ESIC guides it is Form 16. For further you can look in to the attachments. I hope it would help. Stay Cool, Atom.

Pregnancy claim cannot be raised by the company. It has to be initiated by the employee themselves.
Now, under ESI, 86 days' salary is provided to the pregnant woman if she is on leave due to pregnancy, and if she is the member, not the member's wife. Additionally, all expenses incurred during childbirth until discharge from the hospital are covered.

Contact your Regional Director and raise the issue with him. In general, mention the reference number on the accident report and submit it to the local office. If they still do not accept the report, then request a written letter to that effect.
