go to nsdl or uti pan service center in your city with one photo, id and address proof fill form and deposit a fee of 94/- with them write the lost pan card number in application

Dear Sir Form 49a use for new pan allotment but in case of change and correction and duplicate pan card we have to use deterrent form so please find attachment both are here Ashok

No need to worry about that. With the same PAN number, you can apply for another one. I think it will cost Rs. 97. You can go to any UTI TSL center, apply for a new one (Fill CSF form), along with your ID proof and Address proof.
