- Duly completed Form 5 IF. (EDLI Form 5 IF has to be duly completed and submitted by the claimant. The claim form has to be signed and certified by the employer. If there is no employer or the signature of the employer cannot be obtained, the form must be attested by any of the following: Bank manager (in whose branch the account was maintained)
- Death Certificate of the insured person.
- Succession Certificate in case the legal heir files the claim.
- Guardianship Certificate if the claim is filed on behalf of a minor by a person other than the natural guardian.
What is the minimum service period for EDLI?
There is no minimum service period to enroll under EDLI. Can an employer opt out of the EDLI scheme? Under Section 17 (2A) of the Employees' Provident Fund and Miscellaneous Provisions Act 1952, employers can opt out of the EDLI scheme when they take up a higher-paying life insurance scheme for their employees.
The minimum guarantee benefit under the EDLI scheme 1976 is Rs 2.5 lakh if the dead member remained in continuous employment for 12 months prior to his or her death. This life insurance benefit provided to EPFO members is provided at no cost to PF/EPF account holders.
