I am looking to submit a request letter for job relocation. Do we need to attach any medical documents from doctors to support our case?
Additionally, should I include my father's perspective? He resides in a place near the location to which we are seeking the transfer.
Furthermore, should I mention that my current role does not align with my skill set, which differs from my existing expertise, or is the medical condition of my wife sufficient grounds for consideration?

You can write a request letter for transfer on a humanitarian basis. You can attach your spouse's medical record/certificate as a supporting document. You can also add the point that your aged father is alone and this transfer will enable you to take care of him as well.
Lastly, with reference to your statement "current location is not utilizing my skills which are completely different from the skills that I already possess," I suggest not to add this in your letter. It might backfire. Employers' perception will always be different. They might perceive that you were paid for no work or less work. So, it's better to avoid that point.
