Please change the heading of the Google Form. It reads, "Problems of Human Resource Management". The heading gives a feeling that you are conducting a survey on the problems faced by HR Management as a discipline. However, it is a survey on the problems faced by the HR professionals working in the different verticals of the HR Department.
