Whether ESI has been implemented below mentioned area and whether below mentioned area is covered by ESI.
AREA: Thiruverkadu, PIN Code: 600077
On perusal of Form-01 submitted by employer in the system, it is found that the Revenue village/ address is not fully mentioned. To confirm the implementation of employer’s area, employer is advised to submit the Revenue Demarcation Certificate obtained from the VAO of employer’s area mentioning the exact Revenue village where employer’s unit is situated along with employer’s full address and submit the same to the undersigned within 15 days, failing which the code number will be deactivated without any further information.

Dear Colleague,

I am attaching a circular of the ESI Corporation which may be of use for your reference. You may also visit the ESI Regional Office in Chennai, where they will guide you immediately and clarify your doubts.

Regarding the Land Survey number and location records in Tamil Nadu, most of the land records are now digitized. You can download the particulars survey number-wise from the Tamil Nadu Government Revenue Department Website. After obtaining the details, get them countersigned by the VAO and RI of your area, then submit them to ESI. When you contact the Revenue officials and ESI representatives, they will provide further guidance.

For the current letter from ESI, send a request for an extension of about one to 15 days, mentioning that you are working on providing the details as requested after consulting the Authority who sent the notice to you. It would be beneficial to visit the ESI Regional Office to have your doubts clarified at the earliest.
