Hence, here is some advice: you need to clearly state what the benefit is for the people who take the time to answer your questions. For example, you could mention that you will publish the conclusions of your study on Citehr.
I tried to answer your questionnaire and found that the options given are not consistent. For some questions, you have given an option "other." Perhaps, in trying to keep the questionnaire short, you have listed a number of characteristics for some questions. This made it difficult for me to choose an option when only three were given, and the option "Other" was not included.
