Please suggest ways how to make sure that the employees do not take breaks apart from the lunch and break time very frequently.

It completely depends on your setup and organizational policy. For example, if you are working for a software company behind closed doors, you can keep a card access system that can be used in case an employee goes on a break. Excessive breaks can be treated according to your company policy, which should be communicated to all employees in advance.

Adopt a time recording procedure by installing biometric attendance. You need to adopt a system and implement a policy to address the situation if one is not already in place. Make sure to inform everyone about the policy and its repercussions for late reporting in the workplace, with a maximum allowable time relaxation of 5 minutes for each interval.
