Professional Tax & LWF - where can I procure forms for all states?

I want to procure the following forms for all the states (if applicable):

- Professional Tax Remittance Challan
- Professional Tax Return Format
- Labour Welfare Fund Remittance Challan

It would be great if someone can help.

Hi Check with the Law Publishers in the city. Usually they keep the forms for the state and if you are lucky you may get for some other states also. Thanks and Regards
