Please help me with a easy questionnaire which will be filled by a team where on the basis of answers I can identify or nominate employee as leader.

As per your query, you want nominations from the team on their leader, but what is your/company's role in identifying the leader? Ideally, the identification of a leader should be based on actual skills, which vary among individuals. I don't think filling out a questionnaire would serve the real purpose unless you assess actual qualities through observation.
