Dear Prachhaya,
Please note, If you want to give 15 days notice for termination of contract you need to give proper reason for termination along with proper backup documentation. You should not issue any termination of contract till the time you have proper documents.

I have many reasons to terminate him with proper document which no one can deny...I only need the proper format

Why do you need a form?
Just write a letter outlining the offences, and the action you intend to take.
Not sure about Indian labour law, but you need to make sure you are following the correct procedure before you terminate an employee. Not following due process could cause you a lot of legal problems down the track.

Before terminating of employees he has to been served with least 3 showcase letters before for his offence , what type of decision taken by the management that too has to be in written only with authorized signatures . please refer your company standing order once for disciplinary actions against different offences and mistakes committed by the employees.
