am part of an manufacturing firm its hardly 40 employees and want to create a file/or document in which I can maintain the complaints/appreciation or other performance related things at one place which we can refer at the time of appraisal, could anyone suggest how I can create this or if someone have any format for the same and want to share it with me.

In our company we have practice of maintaining physical as well as soft copies of each employee file. We allocate a unique badge no. to each employee and based on that create an employee file when employee joins and file is closed on disassociation with company.

Our file contents consist of:

YELLOW CARD - Summarized info

EMPLOYMENT CONTRACT

EDUCATIONAL QUALIFICATION COPIES

MEDICAL REPORT COPY

JOINING REPORT COPY

SMART CARD COPY / PRINT OUT OF SMART CARD DETAILS - NATIONAL ID

PASSPORT COPY / RESIDENCE PERMIT COPY

GOSI REGISTRATION COPY

SALARY DATA COPY WITH TRADE

SICK LEAVE NOTES

LEAVE APPLICATIONS

PAYMENT OF SETTLEMENT etc.

CLEARANCE CHIT/ NOTE

WARNING / DISCIPLINARY ACTIONS, IF SERVED TO EMPLOYEE

APPRAISAL COPY

EMPLOYMENT REQUEST

INTERVIEW REPORT

DRIVING LICENCE COPY
