Hi, my name is Sridhar, and I worked for 3 years at a US Staffing firm. After leaving the company in September 2015, I discovered that my PF documents had not been submitted to EPFO. Upon inquiring, PF officials informed me that they have not received the forms yet. I am considering filing a case against my former employer in both Civil court and Labor court. If anyone can advise me on the process, I would greatly appreciate it.

If you want to file a case, you need to speak to a lawyer. Before filing a case, however, do read the PF rules.
For a start, what makes you think that it is the duty of your employer to submit your withdrawal forms to the PF department? It's your work. Their responsibility is limited to signing the form and returning it to you.
