Currently, I am in the process of developing my proposal on "The effectiveness of Training and Development on the performance of teachers" using the Kirkpatrick Model. Please guide me on creating questionnaires based on the Kirkpatrick Model. I would appreciate it if you could provide any sample questionnaires related to this topic.

Measurement of training effectiveness is a big challenge, even for regular employees of commercial organizations. The measurement of training effectiveness is difficult on the Kirkpatrick Model. My observation is that hardly any organization measures the training effectiveness beyond Level I of this model.
The challenge of measurement aggravates further for the education sector. Against this backdrop, why did you choose this sector? That I did not know.

As our learned senior member correctly put it, the Kirkpatrick model requires a detailed study for an in-depth understanding. Implementation at higher levels is a rarity, as Mr. Divekar has pointed out.
