I am working in a Pharma company as HR Executive. Our management announce to give an increment to all the employees w.e.f Jan-16.
Our MD sent a mail to all the employees stating that, everyone need to sent their self appraisal containing their achievements & e.t.c to MD through mail only.
Now my question is how to send a mail?? Share your valuable responses along with draft mail.

Dear Suni,

When seniors ask juniors to submit reports, whether of self-appraisal or any other, it is their duty to provide the format of the report. Your MD has done half job. Now Managers could send their self-appraisal report as per their own understanding and it would take whole lot of time for MD to analyse the information and it will not be in the standardised format.

Whether through mail or in hard copy, what matter is a standard format. Standardisation is step towards time management.

Even now also, you may talk to MD and tell him to send a mail stating that the self-appraisal format will follow shortly. Prepare the draft of self-appraisal, show it to him. Let him approve it. Once approved, either let him send mail to all the employees or you send it on his behalf.

Asking for certain information but not sending the format of the information is a typical mistake that happens in the private sector. In contrast, in most of the government organisation, seniors attach format of the report or return.
