We recently faced an incident where a gas cylinder (oxygen & DA) was being carried to the second floor manually. After finishing the work, the empty oxygen cylinder was being transported down to the ground floor by two persons. For this purpose, they tied a nylon rope to the cylinder cap. When they tried to bring the cylinder down the stairs while holding the rope and for hand support, one person got unbalanced on the steps. To balance himself, he grabbed the other person. Due to this sudden action, the rope slipped from their hands, and the cylinder also slipped and fell down on the ground below. Fortunately, the area below was vacant at that time, and no injury happened.
What can be the best solutions for carrying or handling such cylinders?
