Please find PF declaration From newly introduced by EPFO downloaded from PF portal. It seems the said form is in place of Form 11 and Form 13. EPFO has just uploaded this form on portal without any direction, instruction or any circular in this regards. PF officials are unaware about this form, as on date.

Kindly tell us when it will be implemented because recently we put few employees under pf and we took their details in old pf form 2....so whether we need to fill the details from oct 2014 as well in new updated pf form 2.

As on date, there is no guideline on this form by EPFO except it is mentioned in the form at top - "To be retained by the Employer for future reference".
As I said in my first post that it seems the said form is in place of Form 11 and Form 13. It seems to me, this form is not in place of Form 2. As on date, the PF officers are unaware of this form, as I said earlier.

The respective Form is in Place of Form no 11 & Form No 13 but if the Employee is holding UAN Number & if he declare the same the Present Employer will have to link with the same also Pls see the declaration is Signed by Both Employer & Employee
Further A SMS was sent by EPFO to the respective Mobile Number in regards to the respective declaration so in my view there is no notification required
