M. Peer Mohamed Sardhar is a very kind person, please do not misuse him. Like some other bloggers have also said doing a course, conducting research for a project is all learning experience. You are supposed to conduct literature survey, etc and design your own questionnaire and not keep asking for questionnaires suited to your specific needs. We also have to search citeHr or the web to provide you info. I suggest that you help yourself first and then help us to help you by giving as much detail as possible about yourself, the institution where you are studying, the help that you get or don't from your tutor guide, etc.
