So, my quarries are 1. What formalities have to do? 2. What form need to submit to ESIC dispensary or branch office? 3. How does he get the facility?

The employee covered under ESI Act, 1948 who has to visit outstation due to work relating to or in connection with the work of factory can obtain Form ESIC-105 from employer and present the same to nearest ESI dispensary where he is temporary required to work. The blank form ESIC-105 can be obtained from the nearest ESIC Branch Office.
