Dear All,

I have a problem and am not getting a clear concept on this topic. One of our employees' parents is staying in another city and is covered under ESIC. Will he be able to avail of medical benefits for his parents from their local city? The issue is that he has not obtained their ESIC Smart Card to date despite working for the past year.

Sir,

If the employee who is covered under ESI has not prepared and obtained the Family Identity Card, he is likely to suffer as his family will not be in a position to access medical facilities at the place of their residence. Kindly refer to Regulation 95A and Form 4-A (Family Identity Card) as stipulated in the ESI (General) Regulations, 1952.

I suggest that you ask your employee to contact the appropriate Branch Office of ESIC and make all efforts to obtain the ESIC Family Identity Card. The respective ESI dispensary/hospital in the area where the family resides will not provide medical attention to the family until the Family Identity Card is presented to them.

Sir,

You may fill out Form 4-A or complete Form 4, as applicable in your case, and direct the concerned Insured Person to contact the ESIC Branch Office to request the issuance of an Identity Card/Family Identity Card. The appropriate ESIC Branch Office will provide necessary guidance and refer the employee to the authorized agency for the issuance of a Family Identity Card.
